Good review notes preserve the problem, the tradeoff, and the reason for the decision—not just a list of requested changes.
A vague comment creates several different jobs
“Make the headline stronger” can mean a clearer promise to a writer, more visual emphasis to a designer, or a different offer to a product manager. Each person can complete a reasonable interpretation while leaving the original concern unresolved.
Start with the missing information or unclear relationship. For example: “The heading names the service, but the opening section never explains which teams it is intended for.” That observation leaves room for more than one sensible solution.
Keep the problem separate from the proposal
“Move the button higher” is a proposal. “The eligibility explanation and the application action are separated by unrelated content” identifies the relationship at issue. Moving the button might help, but moving the explanation or removing the interruption might be better.
A report or comment should retain the original concern when the implementation changes. Otherwise, a later reviewer can only check whether the requested pixels moved, not whether the communication improved.
Make competing goals explicit
Suppose sales wants a specification sheet behind an enquiry form while support wants customers to reach it directly. Voting on which layout looks cleaner will not resolve the disagreement. The teams are optimizing for different outcomes.
Write down the tradeoff and the decision owner. A possible response is public technical documentation with a separate consultation offer. Another may be justified by the business context. The review becomes useful when the reason is visible rather than buried in a long comment thread.
Leave a decision record, not a comment archive
For a consequential choice, capture what changed or stayed, why, who owns it, and what would cause the team to revisit it. “Keep the public download; reconsider if the document contains account-specific information” is clearer than an unexplained resolved comment.
An unresolved factual question needs an investigation, not an invented answer. Assign someone to obtain the missing information and retain the question beside the decision it affects. This is how review feedback becomes work another person can carry forward.
